Sample Birth Plan

Begin with an introductory paragraph about yourself and who will be with you during labor.

Then write a paragraph reflecting the most important choices you have made. For some women this will be environmental factors, for others it will be about absence or presence of interventions.

The next paragraph will contain other preferences you may have, and a statement similar to: "We understand that some circumstances will prevent us from being able to give birth in this way. If the need to change our plans occurs, please provide us with adequate time to ask questions and weigh our options."

You may want to include a paragraph about how you would like emergencies handled.

Don't forget to add a paragraph concerning the options you have chosen for newborn care.

Your last paragraph can be a short "thanks for reading this" demonstrating your gratitude for any assistance in achieving this goal.
